Please fill out your paperwork online or print it out and bring it with you on your first visit along with your insurance card, driver's license and Physical Therapy script (if you have one).

Insurance
Ark Physical Therapy is IN network with Medicare and Out of Network with all other major insurances. This is necessary for the Physical Therapist to spend more time with each patient. Ark Physical Therapy will submit out of network claims as a courtesy if you have out of network benefits. If you do not have out of network benefits with your insurance company, we offer a reasonable cash-pay rate.
